Seeking help with my first referral letter (TRICAREPRIME)
AS MOST OF YOU ALREADY KNOW MY STORY ABOUT HAVE TO STAY WITH THE NUTRITIONIST UNTIL SHE GIVES ME THE OK FOR THE SURGERY WHICH IS NO GUARANTEE SO I HAVE BEEN ADVISED THAT I AM A CANIDATE WITHOUT THAT CLASS AND TO GO BACK TO MY PCP WIOTH THE FACTS IN THE HANDBOOK FOR TRICARE SO WHEN I GO I WOULD ALSO LIKE TO HAVE MY LETTER TO GIVE TO HER(PCM) SHE IS A CPT SO I KNOW SHE REALLY IS NOT TO MUCH FOR THE SURGERY BUT THAT IS ONE OF THE RESONS WHY I HAVE ASKED AND ASKED AND ASKED SO MANY QUESTIONS TO MAKE SURE I HAVE ALL THE INFO I NEED WHEN I GO BACK TO SEE HER ALSO THAT IS WHY I WANT TO WRITE THIS LETTER SO PLEASE HELP THANKS AHEAD FOR YOUR SUPPORT — JENNIFER S. when i went to my pcp i took the printout on wls in the tricare handbook. i
also typed up a stats sheet on diets tried in the past(with dates, results
and regain), any medical condition(joint pain,rashs ect..),height with
current bmi,and family medical history.

If you want to write a letter stating the medical necessity of having WLS,
there is a terrific generic letter on this site. You can personailize it
to meet your own comorbids, and don't forget to include any comorbids that
you have a tendency to aquire due to family history or other health
problems (for example, arthritis due to MO or heart disease if it runs in
your family). To find the letter, first click on "Your Journey,"
then "Insurance Troubles," next "Getting
Authorization," and finally "Letter of Medical Necessity."
I used personalized forms of this letter to send to both my WLS surgeon and
to my insurance company. Maybe this letter will help to convince your
nutritionist as well. Good luck in your pursuit of WLS. Whatever you do,
don't give up!

i HAVE TRICAREPRIME AND HAD NO PROBLEMS WITH IT. iF YOU GO TO YOUR PCM AND
ASK FOR A REFERAL TO SEE THE SERGON THEN IT WILL BE A LOT EASIER . THE
SURGEON ACTUALLY MAKES THE DECISION ON WEATHER OR NOT YOU ARE A CANIDATE
FOR IT .
