List the name and address of any physician who has ever placed you on a
diet or prescribed medications to suppress appetite...these are
"medically supervised diets". The insurance company will also
accept programs such as, Optifast, Medifast, Physician's Weight Loss,
etc... They do not view Weight Watcher's as a medically supervised
program, but it is also helpful to list programs such as these in order to
establish your ongoing efforts to lose weight. Good luck!

Under my dr's. care I was on fastin, phen-fen, fastin again. This was in
addition to the knowledge she had of me trying weight watcher and richard
simmons deal-a-meal over a period of 5 to 6 years. All these counted a
seperate physician documented weigh loss attempts.

My insurance company states two as well. I used Meridia and a Dietician
was considered number 2.

i am still waiting for approval but if you have ever been on anything that
was later pulled from the shelves, that would help. 50% of my medically
supervised and medicated diets were pulled from the shelves.
