My insurance company told me by phone that they required a psych evaluation
but that they would not pay for it. I slid right past it and did not get
one and they paid. I figured that if it wasn't in writing then they
couldn't force me to do it! I didn't need a shrink to tell me that I did or
didn't need to lose weight or them to tell me to, "Just lay down the
fork!" Good luck!

This should be a piece of cake. I was worried that because I suffered from
depression on a regular basis and that you had to be mentally perfect in
order to be a candidate. I had a psychologist and she just wrote me a
letter stating I was making an informed decision about surgery and that I
understand the risks involved.
