Question:
Does anyone have a copy of an insurance appeal letter?

HELP! Medicare has said they don't want to pay for my surgery which I had in July. They said there was not enough evidence submitted by the surgeon to make them believe that I needed the wls. I had lots of co-morbidities. The hospital's answer is that they probably only sent out the bare essentials to Medicare the first time around. Medicare does not say if they will approve the surgery til after the surgery is done. My surgeon is going to appeal it to Medicare for me. I need a copy of an appeal letter from someone to use as a guideline for my own appeal letter that I am supposed to send to Medicare myself. Can anyone help me out??
