I have Health America HMO. My policy states that WLS is covered only if the obesity is associated with one or more current life-threatening co-morbidities and the surgery is determined to be medically necessare to preserve the life or health of the member. Call their office and ask. I found them to be very helpful, but I'm still waiting for approvel.
