Contact the surgens in your area who deal with insurance companies on a
regular basis. Ask them which ones they have the least trouble with. And
ask the insurance companies specifics about the plans they offer. A small
non-profit I worked for in N. CA did this and we got a great plan for a
reasonable cost.

Hi,
I currently work for CIGNA HMO of NO. Ca., They will pay if you meet the
criteria of a BMI of over 50 with co morbidity, have documented 3 weight
loss programs with in the last 5 years one being with in three months of
application for surgery.
I have Principle Life Insurance PPO and they also pay.
Kaiser has its criteria and has a panel that reveiws requests.
Just remember, each insurance company has a set of criteria. So are further
behind the times than others. Have your personnel or benefits person at
work as these questions when they interview the different sales
representatives from the insurance companies.
Best of luck.
I send you light and love.
