I did not have to do this. I gave my surgeon a (very descriptive)typed up
history of my weight loss attempts, the results, how long I did it, how
much I weighed at various times in my life and he accepted that.

I did not have to follow a pre-diet. I had a very detailed diet history of
my attempts and failures at weight loss as well as confirmation by doctors
I had been to for weight loss. I had comorbidities because of my obesity.
I was approved by my insurance in less than a week. However, I was what is
termed "lightweight". I know that some physicians as their
patients to lose weight before surgery in order to reduce a "fatty
liver".

My PCP had watched my weight go up and down over the years, so when I
talked to him about WLS and told him all of the different diets and
programs I have tried over the years . . . and believe me I tried
everything . . . he signed the paper the surgeon had given me.

Yes, I was. I did go to WW twice in the past 2 years and that was all they
needed to know, I did not have to show any proof which surprized me. I was
aproved after one week for the lap band my BMI was just at 40. Keep the
faith it will happen for you. Best of Luck.

I also went in to my PCP with a detailed letter of all attempted weight
loss in the past and medical proof of obesity problem dating back from the
time I was 15 along with a detailed list of ALL co-morbidites. This was
enough to satisfy my dr and the insurance comapny. They approved me within
2 weeks for a refferal to the surgeon and then in another 2 weeks for
surgery. I had WLS within 3 months of starting the process - FAST TRACK -
my head still spins when I think about it. Good luck on your journey - you
can never be over-prepared!
