Do you have suggestions on writing a weight loss history?
I am trying to be proactive and want to write my weight loss history. Does anyone have any suggestions on how to format the information? I would love any advice on the subject also. — Tina A. (posted on February 20, 2005)

My profile shows how I did mine. The insurance campany never asked me, but
I did go over it at the Dr. office at my initial consult. I just started
with the most recent and worked my way back, and tried to remember the best
I could.

Have you checked out the FitDay.com software yet? IT ROCKS! For a very
small price you can own the software yourself or use some of their limited
features on their website for free. Anyways, it has daily operations such
as: mood log, body log (measurments), weight loss, intake (diet), Diary,
Calendar, & activity log. That is how I am keeping track of my daily
journal! That way my OH profile does not get too big. I just post important
things or weekly reports to my OH profile. HTH!

Dr. Baltasar who is a surgeon in Spain has a downloadable itemized weight
loss list. His web site is www.bodybybaltasar.com - this is for his
patients to fill out - but I'm sure that he wouldn't mind you using it as a
suggestion for your own info. I found it very complete and it helped me to
list my info for another doctor.
