You can check my profile for the letter I wrote - but basically you need to
know what are they looking for? Is this an appeal of a denial? Is this a
personal story of why you need surgery? I think a multi-pager is not the
best, but short to medium with clear and precise information that is what
they are using to evaluate you. If you are trying to supply information to
pass authorization and you know exactly what the criteria are, do it in a
list format. Good luck!

If you are appealing a denial, address only the reason for the denial.
Don't make the letter long and drawn out, and don't make it emotional. ( I
work for an insurance company and we get a lot of "cry me a
river" letters.) Stick to facts, please!

Your note should be from your heart. Do not lie! Express what your life
is like at its current state. Notate what diets you have been on in detail.
This letter should accompany a report that your doctor should send. Your
medical history and help your doctor has tried to help with your weight
loss.
