Question:
Copy of letter to insurance company

If you have a copy of an initial letter from your PCP to your insurance company requesting authorization for you to see a surgeon for reconstructive (plastic) surgery for bodylift, arm lift, TT, breast lift, etc., would you be willing to share this with me?
