How come it dont say anything in my certificate of coverage about having pre-approval

How come they dont say it in the coverage book it just says they do cover the surgery why dont it say all the criteria you have to meet,and you dont know unless you call them and speak to a repersenative,Shoulnt it say that where it says they cover the surgery?    — Jade S.
