Question:
Need a draft of a WLS support letter for my pulmonologist

I have an appointment with my pulmonologist. I just found a surgeon in my area that said I can get a referral from this doctor and my pcp. I would like to be able to take in a referral letter to my pulminary doctor for the surgeon so that if he agrees it will make it easier on him, he'll just have to sign. Does anyone have any referral letters to their surgeon from their pcp and pulminary doc that I could use as a template?    — Nancy S. (posted on April 1, 2003)
