Question:
Help Needed with a General Letter to Insurance Company

I have everything ready to submitt to insurance, but my Dr's office has asked me to attach a "To whom it may concern letter. Any help would be great.    — luckyflowers (posted on March 16, 2003)

Check back with your doc. S/he requested the letter, so find out from him/her (darn, this is an awkward way to write!) exactly what is required.
