Help!! Needing info for Cigna HMO appeal letter!

I am needing help from my AMOS family with my appeal letter to Cigna HMO. I was denied because they say that I do not have 2 26 week supervised diets! What can I do? I cannot afford to do this, and of course insurance won't cover that part of it!! Has anyone ever been denied this and then won an appeal? Or does anyone have any information? I am desperate!
