Yes, I was denied because I didn't have 12 months of consecutive Dr. Monitored diet. However after doing research and talking to Walter Lindstrom (obesitylaw.com) I found that they technically cannot require you to do something that they exclude from paying for in order to qualify for a treatment. It's called basic plain and simple discrimination. I was approved within 3 days of Walter submitting his appeal letter to the insurance company.
