Question:
I'm writing a letter to my ins requesting they approve me, who do I address it to?
I need to know the department...I cannot think of the official name......I dont think "the people who hold my life in their hands" is going to work. lol — cherokey55 (posted on April 10, 2002)

Are you appealing a denial? If not, your surgeon should submit the claim
to insurance. The insurance company wants to have documented medical
reasons for you to have it. You shouldn't have to do anything. If you are
appealing, the insurance company should have given you an address for you
to send the letter to. I wouldn't even 'address' it like you would a
regular letter. I'd put it more in memo form, with a title like Appeal of
Denial and then give them the case number or whatever they used in their
denial letter to identify your claim.

Send your letter to the Medical Director. He/She is the one
who will ultimately approve/deny and consider appeals.
