Question:
Anyone asked to supply proof of family history?
Hi..I was curious if anyone has ever been asked to submit proof of family history? I have a long list of heart disease, diabetes, etc in the family but I don't know how I would ever go about proving it. Just curious! Thanks. — Samantha S. (posted on March 31, 2002)

Hi, Although I wasn't required to submit a family history, I did submit
one. I felt with my weight as high as it was, I was headed in the same
direction my father and other relatives are in now, heart attacks,
diabetes, etc... the list goes on and on. My surgeon seemed to think at my
first pre op appointment that my family history would be the key to getting
me approved. My family history just consisted of a piece of paper I had
typed up stating the relative, their relationship to me, their age,
conditions they had, and whether or not they were obese. No other
documentation was asked of me. Best of luck to you!

Legally, they can't require this. I'm adopted and would NOT have that and
it would be discrimination towards me in that respect.

I was required to submit a family history.There is diabetes and hight blood
pressure and heart problems in my family history. It may have helped me get
approved for the surgery since i don't have any co-morbities-chronic
illness myself.

willing to supply any info that would help me get approved.
