Question:
What chart is used by insurance companies to determine ideal weight?

My policy states they will pay for Gastric bypass if the memeber is 100 lbs. over and it is medically necessary. How is the 100 lbs. determined when all the chart I find has small,medium and large frame. Who determines your frame. The company says nothing about BMI.    — CLAUDIA P. (posted on January 14, 2002)

I went to the website of my insurance company. They had an "ideal weight" table and a BMI calculator on the site. If your insurance company has such a website, you might try there. Good Luck!
