What Are The Specific Medicare Criteria for 'Medical Necessity'?

Where can I find the precise listing for the Medicare Criteria of Medically Necessary? I checked on www.medicare.gov but couldn't find anything about Bariatric Surgery, weight loss surgery or gastric bypass. I KNOW they have a list they go by but where can I get a hold of it?
