Is it possible to be transected AND develop a fistula?

Wondering if my acid and bile reflux could be because of a fistula. I'm transected with 150 cm bypass, almost 9 months postop. I saw my surgeon a few days ago and he prescribed Questran which binds with bile in the digestive tract and exits the body it in a poo. It's a powder that is mixed with water. I had been taking Nexium for 6 weeks prior to this. He wants me off the Nexium for now while I try this drink stuff. I was sure he would have wanted to scope me, instead he prescribed this medicine. It has helped to lessen the severe burning all the way up my throat but I still get the backup of liquids into my throat while sleeping. This backup only happens when I'm sleeping. My question then is this--is the bile and acid backing up through my intestine into the pouch and esophagus or could it be coming directly from the bypassed stomach through a fistula and into the pouch? I haven't lost any weight in 3 months, have been stable at 225-230, start was 302, goal is 160.
