How do you get the insurer to cover the surgery if they have a exclusion policy

I'm a little frustrated. I've had an evaluation with the doctor, and the insurance won't cover the visit, surgery, or anything involved with weight loss. They claim even though it is a chronic condititon, that it is cosmetic and my employer has it as written exclusionary policy. HELP.
