reconstructive surgery for apron, who was approved with Cigna PPO

I just made an appointment with a doctor to do my apron removal and they said it was impossible to get cigna to approve this surgery. I need anyone out there that got cigna to approve the surgery to let me know what I need to do to get approved, and I need to know how long it took you to get approved
