Has anyone recently had surgery that has Federal Blue Cross?

I have Fed BcBs and they do not give pre-approval. I have had to pay my doctor ahead of time and have to put a claim in after surgery and HOPE they pay. Could anyone tell me if they have had recent experience with this insurance and tell me if they have paid. I'm getting nervous since my surgery is scheduled for July 17th. Thanks for any input.
