My PCP gave me a referral for the dr. I am interested in, but Kaiser refused...

In a letter they claimed he is "out of the network". Now here's my issue - I have the Kaiser Signature PPO and it's my understanding I should be able to see any dr. I want, as long as I am prepared to pay 80%. Geeze, I know there'll be a lot of hoops to jump thru in getting the surgery approved, but how can you reach that step if you can't even get in the dr's door? I am in the DC area and am interested in the Lap Band or RNY with Dr. Elariny in Arlington VA. Thanks!
