Sample PCP letters that worked? Aetna?

OK, I had my appointment with my PCP today, who is in favor of my having WSL, and she seemed kind of hassled, so I offered to provide her a skeleton of a referral letter to make things easier. I did this because I have seen offers from some people on the boards of letters that worked with their insurance company, BUT! I don't know how to search the message boards or profiles to find them. So, I'm asking anyone who has one, to send me a letter that worked -- note that I am asking for BPD/DS surgery, with Aetna PPO. Thanks eversomuch in advance to anyone who can help! Diana
