Help! Gained 25lbs in 6 months!

I am 16 months post-op and have started gaining. I feel out of control. I have gained 15lb in the past three months. I know I am eating poorly; snacking, grazing,sweets etc. I just can't get it together and be healthy. I know this is a tool, however would be interested in any advice.
