Does anyone have a PCP referral letter they are willing to share with me?

You can email me directly at [email protected]. I am seeing a new WLS friendly PCP on March 8th and also have my surgical consult on March 8th. I would like to take a pre-typed letter for him to simply sign and send me on my way. I appreciate anyone who can help me. Thanks Laura
