CCN--Does anyone have this insurance company?

The new job is offering us two choices for health insurance, both underwritten by CCN (Community Care Network out of San Diego). They claim to be largest PPO network in the US, but I can't find much on them with relation to WLS. I would appreciate any info at all about CCN and bariatric surgery; I know you have to prove "medical necessity" but is that hard to do? Do they have any sort of protocol list that you have to satisfy before being approved? How long between surgeon's request and approval? I would appreciate ANY info you can give me on CCN. Thank you.
