I've been denied by BCBS IL PPO for

Does anyone have any ideas as to how I should appeal something like this? I have a letter drafted and had been a a Dr. supervised visit as well as gone to the informational lengthy meeting, had tests done, etc., but still denied because of that. Please email [email protected]
