Will the diet you are on for control of diabetes count as a doctor supervised diet?

I have been waiting since Nov. 19 for BCBSFL to approve my request for gastric bypass---first they said the doctor did not send my height and weight--duh!--Now they say I need proof of a doctor supervised diet---I have been a diabetic and under a doctor's supervision for the past 15 years---that includes diet and monitoring of blood sugar levels. In my letter of medical necessity--he told of my attempts to lose weight and none were successful--does anyone have a suggestion as to how this should be worded?
