Does anyone know which hmo covered by NJ medicaid may approve my surgery?

I just recieved my insurance, and medicaid wants me to choose one of these hmo's: Americaid Community Care,Amerigroup New Jersey,AmeriChoice,Horizon Mercy,Physicians Health Services or University Health Plan. If anyone has any input Please let me know.I would appreciate it very much.
