I have to put in 30 day notice, what if surgery date gets moved

I'm not quite to that point yet but I've read alot of postings saying their date got changed. When I have a date I have to put a 30 day notice in for medical leave. What if that changes to a sooner date? You think they will still honor my notice? I know I should ask the HR Dept. but their kind of scatterbrained when you ask them anything.
