Any one have success recently in getting approval for the BPD/DS with Blue Cross PPO?

My friend wants the DS and she has Blue Cross PPO (Calif) who used to approve it, but they now deny the DS due to the "malabsorbtion" part. Has anyone had success recently in getting them to approve the DS? and if so, what documentation was it that got the denial approved? Any info or help would be appreciated. Thanks.
