State of TN John Deere Insurance-Getting Plastic Surgery Approval

I had my gastric bypass almost two years ago. Seen my primary care doctor today for a referral to a plastic surgeon to see about having this very heavy, low hanging apron removed. Several surgeons on the list but when they were call all said that this insurance would not cover even the cost of the consultation and I would have to pay this up front just to see the surgeon. I have letters from three doctors stating that my apron is causing many problems and is medically needed. How should I go about asking the insurance company to reconsider this request? Any and all advice would be greatly appreciated I have no idea where to start and neither does my primary care doctor.
