How would I best compile a letter of appeal?

I was turned down by my insurance company because of a "plan exclusion". I do feel that there is some legislation or some type of information that I can provide this company to convince them that their decision was wrong. How Do I find Tennessee insurance laws? HELP! I just need to be pointed in the right direction.
