What do I need to make sure my surgeon has as far as my history?

I visited the surgeon. He told me he wanted all pertinent obesity/medical history. My Primary Dr. said he would send him all recent labs and sleep studies. The primary said I would have to contact various hospitals that I have had tests done at ( colonoscopy, x-rays, CAts, etc.and any surgical records.) I have had testing and operations done at at least 4 different hospitals and I don't exactly know the dates of all surgeries, etc. Should I just have medical records send everything they have on me? Once I have requested these things be faxed, do I call the surgeons office to see if they got what they needed? I just want to make sure I meet all protocol.
