Has anyone been approved when the insurance company

wants a year of documentation and weight lose with a Doctor? I don't have a whole year and it has been since 1996 that I had a supervised diet plan with a Doctor. Does this mean I will get denied now for sure? Even if I have sleep apnea high blood pressure and diabetic.
