Anyone been approved with UHC EPO???

I have UnitedHealth Care EPO through Dell of Nashville. I contact them about having surgery, they only required that it be medically nessessary. Even though I'm going through the process of determining that it is nessassary (BMI 46.5 weight 288)...I'm still worried that I'm get denied... Anyone worked with them? What else can I do besides working with my PCP to get this going?
