Kaiser Permanente Individual Policy (Oregon)

I've read some posts that suggest that Kaiser has changed its stance on WLS. I'm wondering if anyone has bought an individual policy and was approved for WLS? If so, what was your waiting period and what hoops did you have to jump through to get approved? What surgeries are offered? Do you have to wait for one year before you get the surgery? All information would be helpful. Thanks
