Have I been accepted, denied or slapped in the face????

I received a letter from my insurance company today stating that I was eligible for coverage, but there was a $500 lifetime maximum on "weight reduction". Do they think this surgery is just another diet "program"? How do I reply to this one? Any help would be appreciated!! Thanks!
