My PCP won't refer...

I've just been to my PCP and don't think she will refer. Want to know if anyone in AZ on Aetna USHC HMO could give a suggestion for a PCP who easily referred. Also, only 4 bariatric surgeons in AZ - are any considered in network? If not, do they approve to go out of network? Thanks.
