Anyone used either BlueCross

Have either of these two policies...BlueAdvantageEntrepeneur - -a group plan, or Trustmark--individual policy ever been successfully used to pay for WLS. If so, were you denied and then successful on appeal, or was getting approval relatively straightforward? Thanks
