Does anybody had Humana ppo.....

I am very confused about my insurance, I don't really know if I can go ahead with my surgery, without having a lot of money in front, because I think I have to have the 25%, for the surgery, so my question is...Can I make payments, or I have to have the whole amount in cash...And also they want me to have the surgery in another hospital that my insurance doesn't cover..Vista hospital, Will they accept me?, and if they do, I am sure this is going to be more expensive for me. Please if someone had the same insurance, write me My doctor is DR.Alanis, I live in Katy....Thank you all.......
