Has any one EVER been approved for surgery with John Alden/Fortis insurance?

I was ready to self pay when we got new insurance at work. They have an exclusion, but it is very vague. I have looked and can only find old info on John Alden insurance. I am all for saving money, if I can get them to approve it was save me about 13,000.00! I am also prepared to hire Walter Lindstrom if it would help! Is there any one out there that can maybe shed some light on this?????? All comments welcome. Thanks
