Anyone know a WLS friendly PCP in Southern Illinois?

My PCP is telling me all my problems will disappear with medicine. I've been on Fastins off and on since 04/1999 and now he wants me on Elavil, Restoril, and Flexeril. I simply want to start living. I can barely walk and have went from being 6'0" in 1995 to 5'8" as of 12/14/2002. I weigh 299 pounds and my BMI is 46. I have complete medical records but need more tests done to make sure there are no other underlying problems. He refuses to order these tests that I have requested, or the referrals I need. I live in a small busybody town in Southern Illinois, close to the Indiana border. Does anyone know a WLS friendly PCP who accepts IL Medicaid? I'm desperate. I'm going to apply for disability today, because I cannot function any longer, the pain is too much.
