What companies in Utah have insurance that covers the bypass?

I have IHC. They will not cover this surgery at all. My husband recently was offered UHC of Ut. After looking at this web sight I thought my prayers had been answered. Now I am understanding UHC of UT does not cover this surgery either. I am getting desperate, to the point that changing jobs might be my next step. Any experiences that Utahn's have had with companies that offer insurances that cover this surgery would greatly be appreciated. Also just so you know, if anyone has IHC it's important to call them and appeal the bypass not being covered. That is how Viagra became a covered drug. Keep in mind IHC spend millions yearly on media, advertisments, ect., to let potential members know that they practice preventative care. Makes you wonder why they are so willing to spend so much to tell you that but when you call them to ask if they will help you prevent high blood pressure, diabetes, heart attacks, strokes, sleep apnea, hip & knee replacements, pain management for back problems, ect by covering a simple procedure called the bypass. They respond with "NO". Thanks for any responses.
