how long is normal?

I saw surgeon July 1 and they submitted to insurance on the 15th. Cigna denied on 8/25. They notified dr. by phone but have still not submitted a letter of reason. I have called 3 times and I have called HR at work to complain. Do I have any recourse against Cigna for their lack of communication and professionalism? I know I can appeal their decision but I don't know why I was denied.
