Has anyone used the CHIP BlueCross Plan in IL. to pay for bariatric surgery revison?

I am trying to find out if the CHIP (a Blue Cross PPO) in Illinios, the State Insurance Pool has ever approved a bariatric surgery revision? Or has anyone with a Blue Cross PPO in IL used it to pay for a surgical revision?
