Has anyone in Connecticut dealt with BC/BS century Preferred?

I have seen a physican in Connecticut, who recommends I have the RYN surgery. I've been researching this for 4 months now and have read many articles pertaining to insurance policies. I just need to know what BC/BS CP is requireing as evidence for the need to operate. Any information would help. Thanks
