Lovelace Federal will only pay 50%. I need help with appeal

I am retired USPS and Lovelace Federal will only pay 50% and one day hospital stay and then will be reviewed on daily basis. Reason for 50% is "cosmetic surgery". I need to start appeal. What do I do first? My surgery is this Friday, Dec 14. I will go ahead with surgery and begin the battle as soon as I am home. Thanks,
